No two people are the same, so why should your
health care be?
That’s why we’re supporting a patient-centered
approach that puts you at the center of your
health care. That means focusing on preventing
health problems in the first place. How? We’re
giving PCPs an even more important role through
patient-centered care. And we’re giving them
the right information, tools and support so
they can manage your care based on your own
health history.
We hope to help improve your health and care and
lower your costs through:

Better access:
You’ll be able to go to your PCP
when you need to – after hours, on the weekends
or even on the web. And your PCP will have the
time needed to give quality care.
•
Lowering health risks:
That means your PCP
will have information that can identify when you
haven’t had recommended treatments. Your PCP
will be able to take care of any gaps in care (like
a missed eye exam) and help keep you out of the
hospital or emergency room (ER) more often.

Coordinating care:
PCPs will have the tools
and information they need to give the best care.
They’ll be able to handle more of your care,
instead of referring you to specialists. They’ll
still be able to send you to specialists for more
care when needed, but they’ll work together as a
team. And we’ll even alert your PCP when you
are admitted to the hospital or go to the ER.
•
Meaningful information:
Your treatment will be
based on your specific health needs. We’ll give
PCPs the medical, drug and lab data they need
to make treatment recommendations based on a
full picture of your health.

Sharing wellness and preventive care:
We’ll offer
our care management nurses, who can work
as part of your PCP’s team, to help manage
situations that require complex care.
•
More convenience:
Since you should need fewer
visits to get the right care, you could save time
and hassles.
